Important Contribution and enrollment information will be mailed out by the Fund; if you don\u2019t complete and return this information on a timely basis it could negatively affect your ability to enroll or qualify for coverage with the Fund. If you have moved within the past year or you are new to the Health and Welfare enrollment process, <strong>PLEASE verify your address with the Fund\u2019s administrative office<\/strong>. <!--more--><\/p>\n<p>To verify your address you may call (818) 243-0222 and ask to speak to one of the Fund\u2019s member service representatives. However, and in order to ensure accuracy, we do request that change of addresses be in writing. You may send your change of address request to:<\/p>\n<p style=\"text-align: center;\">Professional Musicians Local 47 and Employers\u2019 Health &amp; Welfare Trust Fund,<br \/>\nc\/o PacFed Benefit Administrators<br \/>\n1000 North Central Avenue, Suite 400<br \/>\nGlendale, California 91202<\/p>\n<p>Please include your name, new address, telephone number and the last four digits of your Social Security number to allow us to properly identify you in our system.<\/p>\n<p>Although you may have notified the Union of an address change, that information is <span style=\"text-decoration: underline;\"><strong>not<\/strong><\/span> routinely passed on to the Fund, so please make sure that you separately update your mailing address information with the Fund\u2019s Administrative Office.<\/p>\n<h2>ELIGIBILITY NOTIFICATIONS<\/h2>\n<p>October 2 is the end of the Fund\u2019s qualifying period for determining eligibility for the coverage year January 1 through December 31, 2019. The Fund will then make its eligibility determination in early November. Once eligibility has been determined, enrollment information will be mailed to the participants.<\/p>\n<p>All eligible participants must respond as soon as possible, as delaying the submission of your enrollment form and co-premium may affect your coverage and benefits.<\/p>\n<p><strong>If you are out of town during the months of November or December, please make arrangements to ensure that this information does not remain unopened until you return.
