{"id":1786,"date":"2015-09-16T23:06:24","date_gmt":"2015-09-16T23:06:24","guid":{"rendered":"http:\/\/www.afm47.org\/press\/?p=1786"},"modified":"2015-11-16T23:10:40","modified_gmt":"2015-11-16T23:10:40","slug":"hw-fund-who-is-collecting-on-your-health-insurance","status":"publish","type":"post","link":"https:\/\/afm47.org\/press\/hw-fund-who-is-collecting-on-your-health-insurance\/","title":{"rendered":"H&#038;W Fund: Who is Collecting on  Your Health Insurance?"},"content":{"rendered":"<p><em>Professional Musicians, Local 47 and Employers\u2019 Health &amp; Welfare Fund<br \/>\n<\/em><br \/>\n<strong>Who is Collecting on Your Health Insurance?<\/strong><\/p>\n<p>The Professional Musicians Local 47 and Employers\u2019 Health &amp; Welfare Fund (\u201cFund\u201d) provides a $20,000* Life Insurance benefit to Participants who have qualified for benefits.<\/p>\n<p>When you qualify for the Health benefits offered by the Fund, you also qualify for the Prudential Life and Accidental Death and Dismemberment benefit. While you must complete an enrollment form and pay the required co-premium to be enrolled in and covered by the Fund\u2019s health benefit plans, there is no such enrollment or co-premium required for coverage through the Fund\u2019s Life and AD&amp;D benefit program. Once you become eligible, you are automatically enrolled.<!--more--><\/p>\n<blockquote><p>Update your life insurance beneficiary form with the Musicians Health &amp; Welfare Fund<\/p><\/blockquote>\n<p>If you have qualified for benefits through the Fund and do not remember completing a beneficiary form or if you\u2019ve had a change in your life, (marriage, divorce, birth of a child) you should consider completing and submitting a new form.<\/p>\n<p>You may download a beneficiary form from the Fund\u2019s website <a href=\"http:\/\/www.pacfed.com\/musicians\" target=\"_blank\">www.pacfed.com\/musicians<\/a> (select the \u201cforms\u201d link on the right-hand side of the page), or you may call the Fund\u2019s Administrative office at (818) 243-0222 and request that one be mailed to you.<\/p>\n<p>When you have completed your beneficiary form mail it to:<\/p>\n<p>Professional Musician\u2019s Local 47 and Employers\u2019 Health and Welfare Trust Fund,<br \/>\nc\/o PacFed Benefit Administrators<br \/>\n1000 North Central Avenue, Suite 400<br \/>\nGlendale, California 91202<br \/>\n<strong>WHICH HEALTH I.D. CARD TO USE WHEN &amp; WHERE?<\/strong><\/p>\n<p><em><strong>Medical and Prescription Drug Benefits<\/strong><\/em><br \/>\nPresent your medical plan I.D card (Health Net or Kaiser) when visiting a medical doctor or when filling a prescription prescribed by a medical doctor.<\/p>\n<p><strong><em>Landmark Chiropractic and Acupuncture Benefit<\/em><\/strong><br \/>\nWhen obtaining services from a Landmark chiropractic or acupuncture provider, advise the provider that your benefits are provided through the Landmark health plan. The provider will contact Landmark to determine benefit and eligibility.<\/p>\n<p><strong><em>Medical Eye Services (MES Vision)<\/em><\/strong><br \/>\nWhen seeking vision services through MES Vision you must obtain a claim form and submit it to the provider. The provider will contact MES to verify your eligibility and to determine the level of available benefits.<\/p>\n<p>You may download an MES claim form from the Fund\u2019s website,<br \/>\nwww.pacfed.com\/musicians (select the \u201cforms\u201d link on the right-hand side of the page), or you may call the Administrative office at (818) 243-0222 and request that one be mailed to you.<\/p>\n<p><strong><em>Dental Benefits through Delta Dental<\/em><\/strong><br \/>\nDeltaCare (DHM): When obtaining services from a DeltaCare provider present your DeltaCare I.D. care. The dental office will contact Delta Dental to verify your eligibility and benefits.<\/p>\n<p>Delta Dental (PPO): When seeking services advise the dental office that your benefits are provide through Delta Dental. The dental office will contact Delta Dental to determine the level of benefits and eligibility.<\/p>\n<p>Note: To obtain covered services for medical, chiropractic\/acupuncture, vision or dental you must be eligible and enrolled in those benefits.
